Just print this page on on letter sized paper or cardstock (8.5x11”). Cut along the solid lines and fold along the dotted line. You can even color the card in if you’d like.

This is a digital download, so no item will be shipped to you. You can keep the file forever and print as many as you need. Print at home on your own printer or send to your local office supply shop or printer.
